GLOSSARY
Schedule - a list of specified amounts payable as certain benefits.
Service Agreement - also called the Evidence of Coverage and Health Service Agreement, this is the official plan contract. It explains in full all the terms of coverage, including benefits, co-payments, how to access coverage, and the plan's exclusions and limitations.
Settlement options - choices available to the insured/owner for distribution of insurance proceeds either to the insured or the insured's beneficiary, instead of an immediate cash lump sum payment.
Short-rate cancellation - canceling when coverage has been in force for less than one year with a less than proportionate return of premium.
Stop-Loss - the limit to the individual's liability for co-insurance.
