Shield Life Short Term
Insurance Option 1 and Option 12
Eligibility: Must have resided in
the U.S. for the past six consecutive months, or be permanent residents
("Green Card" holders) or US citizens. Must be under 65 years
old for Option 1. Option 12 policies may be issued up to 64 years and 6
months of age. Dependent children must be at least 15 days old, but under
age 25 (unless disabled), unmarried and rely on the applicant for support
and maintenance. Must meet all health requirements as indicated on the
application.
