Provider Search
Use this page to find providers participating in networks, which have
agreements with your insurance company. While most plans allow using of any
licensed provider, we encourage you to find a participating one. Participating
providers usually agree on certain fee schedule and may bill insurance company
directly, thus potentially decreasing your out-of-pocket expenses and
administrative hassles. Moreover, some insurance companies may decrease or even
waive deductible, if you go to a participating provider.

The PPO network is organized and administered by The First Health Network.
To get the best results from your search, please enter as many criteria as
possible. The more fields that have entries the narrower your answer will
be. IF YOU ARE A MEMBER OF THE IMB-SBC, WITH GROUP NUMBER 602, THERE
MAY BE ADDITIONAL PROVIDERS IN YOUR AREA THAT ARE NOT LISTED IN THIS

| The SOS Worldwide Network is on standby around the clock,
24-hours a day, 7 days a week, with doctors, coordinators and a world of
resources. For medical emergencies, please call your nearest SOS Alarm
Center. SOS Members can have access to the services available at SOS
Clinics, subject to the terms and conditions in force at each respective
clinic. |

throughout the world, and this network is growing daily. Many of your
provider selection decisions will be based on geographical location,
provider specialties and, in most cases, selecting a provider from the
networks below will reduce your out-of-pocket expenses. |

| For a list of participating facilities and doctors in SRI's
U.S. network, search using the link above*. Or, contact SRI Assist at
800-335-0477 or 317-575-2656 during normal business hours. Once you have
found a facility and have set up an appointment, contact SRI Assist with
the appointment information. SRI Assist will then send a notice to the
facility outlining your coverage information and program details. While
using the SRI Network does not guarantee benefits, it allows SRI to
streamline the administrative process for both you as well as your
physician.
